Correction of mesial bite in a teenager
We are being seen by an orthodontist in Magnitogorsk. According to the results of the last examination there are several opinions: 1. it is necessary to remove 3 permanent teeth before installing a bracket system; 2. to install a bracket system without removing teeth, but then there is a possibility of extending the lower jaw during the eruption of wisdom teeth; 3. do nothing until the age of 15 years. Which option is better?
The doctor's answer
Based on the data provided, we can note that you are characterized by:
- Bite open in the anterior region;
- Mesial bite;
- Vertical type of growth of the facial skeleton;
- Severe crowding of teeth in the upper and moderate in the lower jaws; mainly a lack of space for 15 (retinated tooth from top right), 23 (upper left canine) and 33 (lower left canine);
- Tooth retention 15 (delayed eruption);
- The displacement of the center of the upper dentition along the face by 1 mm to the right.
Such a situation can be solved in the following ways:
- Correcting a mesial bite by moving the lower teeth backwards - this requires the space in the tooth row that we can get as a result:
- distalization – when the entire dentition is moved backwards (up to 2 mm on each side): micro-implant support is required.;
- separation – grinding the teeth in a safe volume, as a result of which they are reduced in size and free the necessary space (up to 2-3 mm on each side);
- removals – as a last resort.
- Correction of open bite – extrusion of incisors (extending incisors to meet each other to close the bite and create an overlap) and / or introduction of upper molars (the last large teeth), possibly with the support of micro-implants (there is an age limit).
- Alignment of dental rows with or without tooth extraction and surgical relocation of jaws with correction of mesial and open bite (after 21-25 years of age).
- Creating space for the upper left canine 23 and retined premolar 15 on the upper right
- removal;
- distalization;
- forward tilt of the front incisors;
- separation;
Recommendations:
- There is a high probability that teeth should not be removed.
- The growth of the mandible will continue regardless of the treatment plan until the age of 21. After completion of orthodontic treatment, retainers will be needed for a long time.
- Treatment should be started now, while there is still a chance for bone remodeling. After 15 years of age, there is a high probability of treatment with surgery.
