Why is professional oral hygiene necessary?
All oral diseases develop through poor oral hygiene. Plaque builds up on the teeth – these are microorganisms that live on the surface of the tooth and contribute to the development of diseases. Over time, soft plaque turns into calculus. Tartar builds up and goes under the gum, thus developing periodontitis, subgingival calculus and bone destruction.
The main thing in professional dental cleaning – is to remove plaque and tartar and strengthen the enamel.
Plaque cannot be removed 100% at home, it is still deposited somewhere on the sides of the teeth. If there is crowding or gaps between the teeth, you can't clean these areas at home. We have not met a single person who can perfectly clean their teeth themselves at home.
Visual diagnosis of the oral cavity
At the first stage, the hygienist conducts an examination, determines the general condition of the teeth and periodontal tissues. He or she will ask the patient how he or she takes care of the oral cavity, what brush and toothpaste, how often and with what movements he or she brushes his or her teeth, whether he or she uses floss and rinses. Whether the patient smokes, likes coffee and strong tea. It happens that the patient carries out regular and thorough hygiene and does not have special habits, but due to bite abnormalities plaque accumulates faster. This study makes it possible to make recommendations for the patient: what hygiene products to use at home and how often to do professional oral hygiene.
Determination of the oral hygiene index
We stain the teeth with a special dye that shows up purple on teeth with plaque, and the more severe the impurities – the brighter the color. This allows us to show the patient where he does not finish brushing, to explain to him that he can not perfectly clean his teeth at home, to recommend additional means of personal hygiene. This is most often relevant for patients with braces. The dye can be easily washed off.
Plaque removal
Sandblasting removes pigmented plaque. It does not remove tartar.
In the «Konfidencia» clinic, we use a powder blasting machine called PROPHYflex. It delivers a pressurized jet of water mixed with calcium powder, unlike the Airflow technique which uses soda powder. Calcium powder – is chalk, it does not destroy enamel and actively removes all soft pigmented plaque. Its granules are round, and unlike baking soda powder, they do not scratch the enamel.
Ultrasonic teeth cleaning
Ultrasonic brushing removes tartar – are hard dental deposits.
The ultrasonic wave vibrations of the handpiece crush the stone and remove it from the surface of the tooth. We shrink it, crush it and remove it from the surface of the tooth.
Tooth polishing
After all the soft plaque and tartar has been removed, we polish the teeth. Polishing is done with special polishing rubbers or brushes and pastes. The pastes come in different flavors for the choice of the specialist and the patient. The enamel surface after cleaning is porous, rough and sensitive to external influences. Polishing is necessary to prevent bacteria from settling on the surface of the teeth and to make the teeth smooth.
Fluoridation
After polishing, a fluoride application is made. This process is called remineralization of the teeth. It is necessary to strengthen the enamel of the teeth and, as a result, prevent tooth decay.
Remotherapy is performed with a special gel with trace elements based on fluoride or calcium. Fluoridation of teeth is performed in the hygienist's office. If it is necessary to strengthen the enamel with calcium, the hygienist will additionally recommend home remotherapy based on calcium, as calcium is assimilated with longer use, within 2-3 weeks.
In the procedure in the clinical setting, fluoridated gel is applied in a mouth guard. The patient bites into the mouth guard and sits for 1 minute. After the gel is distributed with the tongue over the teeth. It is recommended not to drink or eat for up to two hours.
Selection of special home oral hygiene products
Every patient has a different tooth structure and bite, may have orthopedic or orthodontic structures, implants or composite fillings. We all have our own habits. Depending on these factors, a set of home hygiene products is selected.
For orthodontic patients, there are special manual brushes and brushes to carefully care for teeth and gums with braces. These patients also need enamel strengthening and calcium pastes are recommended.
For orthopedic, orthodontic and surgical patients, an irrigator is indispensable – a special device that removes plaque and food debris between the teeth and structures, massages the gums and improves their condition by means of multiple water pulsations.
For some patients, we recommend electric toothbrushes for more thorough care. For some, toothpastes with fluoride are suitable, for others without. Some for sensitive teeth. Hygiene – a very individual thing.
Recommendations after brushing
- After applying fluoride, you should not rinse your mouth, drink or eat for up to two hours.
- If anesthesia was administered during hygiene, you should not eat until tooth sensitivity returns.
- For a day it is recommended to exclude all coloring foods (beets, carrots, blueberries) and drinks (strong tea, coffee, red wine). Because after cleaning the enamel is the most susceptible to all colorants.
- After professional hygiene, be sure to replace your toothbrush with a new one to prevent germs from re-entering your mouth.
- When the patient has gingivitis, an inflamed gum, the procedure is accompanied by bleeding gums. This can last up to two days. In such cases, we prescribe an anti-inflammatory rinse for the patient.
- The main recommendation – quality home hygiene.
Additional home care
When the enamel is weakened, bright white spots appear on the teeth. These are pockets of enamel demineralization. This means that the teeth are lacking calcium.
In this case we prescribe home remotherapy with calcium. There is a special gel that the patient applies after brushing teeth at home at night. This is necessarily done for a course of at least two weeks, maybe more. It depends on what kind of foci. It is applied either in a mouth guard or on a brush and left on the teeth overnight.
It is important to eliminate fluoride toothpaste before applying the gel. Fluoride and calcium displace each other, and if you brush your teeth with a fluoridated toothpaste, the fluoride will create a protective coating on your teeth that calcium cannot penetrate.
Professional hygiene for children
For children, there are no peculiarities in the procedure of professional hygiene. With children, the main approach is the following: if the child is small or it is the first time at the procedure, doctors start small: brushing with a brush and paste, for example – and watch the reaction. If the child is calm about the process, PROPHYflex can be added. For tartar removal in the case of children, hygienists try to use hand instruments instead of ultrasound.
Professional hygiene for orthodontic patients
In the clinic «Konfidencia» there is a separate complex of professional hygiene before the installation of braces. Its difference is that at the end of the procedure, instead of fluoridation, Stomysens procedure is done. Dry teeth are covered with varnish with special particles of hydroxyapatite, which are embedded in the structure of enamel for its protection. This procedure can be done additionally to strengthen the tooth enamel, if desired or necessary, for any patient.
The peculiarity and complexity of hygiene of patients with braces is that the braces block access to all areas of the teeth. The hygienists of our clinic remove all the brackets and elastic bands before the procedure and, if the patient does not go to the orthodontist immediately after hygiene, put them back on. The patient remains in the chair with only the braces open. This gives an opportunity to clean everything thoroughly.
Professional hygiene for pregnant women
There is a common myth that it is not recommended for pregnant women to have ultrasound cleaning. It isn't. Since the ultrasound wave does not propagate, does not reach the baby, to the fetus.
