Virtual set-up of treatment with aligners

Virtual Setup — is a simulation of the result of orthodontic treatment. It allows you to trace every tooth movement in sequence and see the final result before you even start the treatment..

Even before the introduction of 3D technology in dentistry, orthodontists tried to model tooth movement on physical sets. They took impressions of the patient, cast plaster models, carefully sawed the models into segments for each individual tooth, and used melted wax to manually place the teeth in the correct position. It was a very painstaking, but alas, very inaccurate process. But even then, orthodontists understood the value of the set-up — visualization and prediction, which are very important for the patient to make a decision.

Technology has now reached a whole new level of accuracy and predictability. Setups have become as detailed as possible and allow for modeling not only the movement of the crown (visible) part of the tooth, but also the roots through integration with the 3D CBCT X-ray. Thanks to the availability of a 3D scanner in the clinic, the entire process of taking an impression, modeling and approval of the set takes only a few days.

Advantages of computer modeling

Advantages:
  • Safety of treatment

  • Reducing the risks of complications

  • Comprehensive smile planning

  • Inspiration for the patient

  • Predictability of treatment

Disadvantages:
  • The clinician must have a good understanding of the biomechanics of tooth movement for proper set-up planning

Treatment safety and risk reduction

The main risks of orthodontic treatment — complications and side effects — arise from improper planning or lack thereof. Orthodontics — is not a cosmetic movement of teeth, but a serious intervention in the dento-mandibular system. If you start treatment without clear calculations and understanding of what and where to move, you can get unpleasant consequences: resorption (destruction) of tooth roots, gum recession, joint dysfunction. Therefore, before each treatment, no matter – on braces or aligners, the orthodontist must carefully plan everything and assess the risks of future tooth movements.

The virtual set-up is a good tool for preventing complications. Thanks to the high quality of the software on the set-up, it is possible to evaluate root movement, bone volume, and final tooth alignment in detail and to choose the optimal treatment plan.

Comprehensive smile planning with surgeons and orthopedists

A common reason for a non-ideal final result — is a lack of communication between the related specialists. Most of our patients do not come with only orthodontic problems: they do not like the shape and color of their teeth, they have missing or severely decayed teeth. Therefore, after teeth alignment, they will need implants, crowns or veneers. In order for the orthopedic restorations to be aesthetically pleasing, functional and installed with minimal preparation, the orthopedist and orthodontist must consider the final position of the teeth and restorations in advance.

The virtual set-up makes communication very easy. Doctors can not only verbally discuss the treatment plan, but can see the final position of the teeth, discuss the movements and adjust the treatment plan if necessary. Also, in some programs, the position of the TMJ can be taken into account when creating a set-up and even simulate a splint to treat the dysfunction.

Inspiration for the patient

For patients, the setap — is an emotional motivational tool. Many of them have never smiled widely because of shyness and insecurity. They can't even visualize themselves with a beautiful smile. And significant tooth movement on the setape is very inspiring and motivating to start treatment.

In addition, the set-up provides the patient with a clear timeline for treatment and the number of drops. The patient can plan his life or further treatment with other specialists. It should be clarified that with treatment on aligners there may be corrections — additional mouthguards that were not part of the original plan. Usually the orthodontist warns about the need for corrections and also demonstrates them on the set-up.

Setup creation process
1
Orthodontic Diagnosis. Prior to any orthodontic treatment, the orthodontist collects diagnostic data: photoprotocol, CBCT image, dental scan, examination
1 hour
2
Making a treatment plan. Based on the diagnostic data, the orthodontist thinks through all the tooth movements and analyzes how they will affect the bone tissue, soft tissue, and tooth roots. This is a complex intellectual work that requires experience and good knowledge of biomechanics.
up to 2 weeks
3
Writing the specifications for the set-up. The doctor writes the most detailed instructions on where and how much to move teeth down to the millimeter and sends it to the dental technician. The quality of the set-up depends on the accuracy of the wording.
up to 1 week
4
Setup creation by the dental technician. The technician models the set from scratch — segments the teeth on the virtual scan as separate objects and moves them step by step into the desired position according to the instructions from the dentist.
up to 2 weeks
5
Adjustment of the set-up. The orthodontist must check the finished set-up. What needs to be checked: the position of the roots of the teeth — whether they do not protrude beyond the bone; whether the final shape of the smile suits the patient; whether the planned movements are possible from the biomechanical point of view.
up to 2 weeks
6
Sending the set-up to print after coordination with the patient.

An overview of technologies for creating treatment setups

Spark's Approver program

Approver software was created by Ormco to simulate orthodontic treatment sets on Spark aligners. It is one of the newest manufacturers of aligners.

The simulation program has many possibilities for very detailed planning of safe treatment taking into account all biological parameters:

  • Integration with the CCT snapshot,
  • Visualization of the roots of teeth and their movements,
  • Table of movement of each tooth in each plane up to fractions of a millimeter,
  • Guides for teething during treatment on children's aligners,
  • The ability to plan the installation of occlusion pads, hooks and attachments.

Physician's Review

Leading orthodontist

The program allows you to create detailed sets that are as close as possible to the actual movement of teeth. This gives the dentist and the patient confidence in the quality of the result.

Program from Eurokappa

Eurokappa — is a Russian manufacturer of aligners, which is not inferior in quality to foreign manufacturers.

What our orthodontists note when working with Eurokappa sets:

  • The program has integration with CLCT,
  • Dental technicians are orthodontists, so setups usually require fewer adjustments,
  • Setup production time — 2 days.

Physician's Review

When planning sets at Eurokappa, I like the high speed of development, discussion and approval. I am very happy that my patients can quickly see the set-up, receive their mouthguards and start treatment.
