The Marco Rosa appliance – is a fixed orthodontic appliance for the expansion of the upper jaw. The fixed plate is also called Haas appliance – in honor of the German orthodontist Andrew J. Haas, who created and first began to use the design for the correction of malocclusion in children. At the beginning of the 21st century, the palate expansion appliance was modified and improved by Italian orthodontist Marco Rosa.

Apparatus construction

The plate for rapid palatal expansion consists of:

  1. Screw

  2. Plastic base

  3. Metal rings for fixation on temporary molars

  4. Metal appendages for fixation on milk canines

In addition, the orthodontist can add other elements – hand-shaped spurs, pushers, hooks on rings and others depending on the additional tasks of orthodontic treatment.

The Marco Rosa appliance is custom-made using an impression or scan of the jaw. The orthodontist creates the design based on the clinical picture and passes the information to the dental technician. Each appliance – is not a standard, individualized piece of work.

The color of the plastic base, patterns, glitter and other embellishments are chosen by the young patient.

How does the Marco Rosa device work?

The main task of the device is the expansion of the upper jaw. It occurs by activating a screw in the center of the structure. Turning the screw pushes the plastic base apart, which in turn exerts pressure on the dentition.

In which cases is the Marco Rosa device prescribed?

The device is most effective in early replacement bite – from 6 to 9 years old, and is prescribed in clinical situations, for which it is necessary to expand and normalize the shape of the upper dentition:

  1. Crowding of teeth;

  2. Lack of space for eruption of permanent teeth;

  3. Narrowing or shortening of the upper dentition;

  4. Insufficient development of the upper jaw;

  5. Cross, distal or mesial occlusion.

What results can be achieved with the Marco Rosa device?

  • After the expansion of the upper jaw, conditions are created for the correct position and development of the lower jaw;

  • Due to the expansion of the upper dentition, the tongue occupies the correct position when swallowing, chewing and speaking, which prevents the formation of skeletal malocclusion pathologies;

  • A place for permanent teeth is created, which reduces the risk of crowding or pronounced dystopias of teeth in a permanent bite;

  • Stimulating growth ensures the harmonious development of facial features in a child;

  • The duration of orthodontic treatment in permanent bite is reduced.

How do I activate the Marco Rosa screw?

The main feature of the appliance — the need to independently tighten the screw. The orthodontist gives the parents a special key, which is inserted into the screw hole and screwed a quarter turn. The screw must be tightened at intervals that the orthodontist prescribes.

Advantages of the device

Advantages:
  • The appliance is fixed – the child will not forget to wear it and will not lose it. The fixed appliance works permanently in the mouth, which improves its effectiveness and shortens the treatment time compared to removable plates.

Disadvantages:
  • For the first few days, the child may experience discomfort – the plastic structure on the palate can impair diction and lead to increased salivation. Children adapt very quickly, and after a few days the appliance will not cause any discomfort.

Care features of the appliance

The appliance practically does not impair oral hygiene – the child should brush teeth as usual. But the plate itself and the screw on the palate – this is an ideal place for food residue or plaque accumulation, so it is recommended to rinse the mouth well with water after meals, and at home with the help of parents to use an irrigator.

Digital fabrication of the Marco Rosa appliance at «Confidentia»

To ensure that the appliance is properly fixed in the mouth, an accurate impression of the patient's teeth must be taken. The doctor tries on an impression spoon, kneads the mass and «imprints» the teeth and palate. Taking impressions for children can be an unpleasant procedure – the impression mass must cover the teeth and the entire surface of the palate, which can cause a gag reflex. In addition, you have to wait 2-3 minutes for the final solidification of the impression. Not every young patient has the patience and self-control to endure such an unpleasant procedure.

At the clinic «Konfidencia» we use a more modern approach instead of impressions – dental scanning. The scanning procedure is completely safe and much more comfortable for patients. The orthodontist uses a small intraoral camera to «take a picture of the teeth within 2-5 minutes, and an accurate 3D model of both jaws appears on the scanner screen. The digital model is then sent to a dental laboratory for modeling the appliance.

What are the advantages of scanning over taking impressions?

  • Scanning is a more comfortable and safe procedure.

  • The scan does not cause a gag reflex. At any time, you can stop without losing the result and give the child a rest.

  • Young patients see the scan as a game – their teeth appear on the scanner screen during the process and they are very excited to see the result and their teeth from the outside.

  • The digital model of the teeth is more accurate, reducing the risk of a poor fit and the need to adjust the appliance.
