Is it possible to align teeth without extractions?
I am looking for an orthodontist. Is it possible to get a good result without extracting teeth? 3 eights have been extracted, the one on the top right is in the gum. I am afraid that because of the crowding of teeth, the upper jaw will move forward after treatment and the bite will be unsightly.
The doctor's answer
Based on the data provided, we can note that you are characterized by:
- Moderate crowding of teeth in the lower jaw;
- Moderate crowding of teeth in the upper jaw;
- Deep bite;
- Distal occlusion;
- Narrowing of the smile and displacement of the center of the lower dentition to the left by 3 mm.
- Retention of the 18th tooth (top right).
Answer to your question:
The decision to extract or not to extract teeth is made only after a thorough analysis of all diagnostic data (in addition to intraoral photographs, we also need facial photographs, 3D X-rays of your skull and plaster models of your teeth).
What we can say, however, is that based on your photos, IF tooth extraction will be necessary, it will be only on the upper jaw and symmetrically on the right and left, as the center of the upper dentition is centered on the face.
The position of the upper jaw does not change during treatment (teeth move). During treatment, the teeth will change their position, but we predict their position and relationship (bite) at the diagnostic stage (i.e. before orthodontic treatment).
This situation can be solved in the following ways:
- Moving the lower teeth forward – increasing the inclination of the incisors. Restriction: in this case, we are limited by the amount of bone in the anterior teeth. It can only be assessed by 3D-scan.
- Moving the upper teeth back - this requires the space in the tooth row that we can get as a result:
- tooth extraction;
- distalization – when the entire dentition is moved backwards (up to 2 mm on each side): micro-implant support is required;
- Separation – grinding the teeth in a safe volume, as a result of which they are reduced in size and free the necessary space (up to 2-3 mm on each side);
- combination of the above methods.
Restriction: inclination of the upper anterior teeth – it is necessary to assess the initial inclination on x-ray 3D-scan; also part of the freed space should be spent on leveling the crowding.
- Combining 1 and 2 methods.
Possible treatment options:
We can offer you several possible solutions. In terms of efficiency, price/invisibility compromise and taking into account your situation, we would recommend you to be treated with a combined bracket system: Damon Clear ceramic braces on the upper jaw and Damon Q metal braces on the lower jaw.
Please note that these recommendations are for informational purposes only. You can get a detailed consultation only at the appointment of an orthodontist after examination and diagnosis.
